"Fail-safe" system for diagnosis quality control in cancer screening

Short project presentation

The project proposes a complex system able to monitorise both women population with high risk for genital and breast cancer found in a screening program and cancer patients. The Babes-Papanicolaou smears are taken for the cervical cancer screening. In order to early detect cervical cancer we use the conventional technique that is used for the smear and for women found with dysplasia a second smear, in monolayer technique is taken and is followed by HPV.
The study of BRCA genes in the grade I relatives of patients with breast and ovary cancer and their monitoring will contribute to decrease of mortality by breast and ovarian cancer to the risk groups by early detection and improving survival and the quality of life. The computer system has the databases with tested women, high-risk women and the algorithms for individual diagnosis and therapeutic options. The fail-safe system assures the diagnosis and treatment quality at European standards.
CanScreen project proposes a complex system based on modern medical investigation methods, last generation communication equipment that use ICT connection technology of resources at different levels.

Project main objectives

Cancer control represents the transfer of knowledge issued from research into strategies and actions that can rise the survival and quality of life to those affected by disease. By definition, cancer control includes any activity that can reduce morbidity or mortality by cancer. The knowledge and control of cancer are limited. These assumes scientific knowledge and experience starting from the complexity of intracellular molecular rules to individual life style. Meanwhile it requires a competent leadership and the best use of resources for planning, implementing and assessment of control strategies. The goal is reducing morbidity and mortality by cancer and improving of quality of life for patients with cancer and also to their families worldwide where the cancer morbidity is high or where epidemiological indicators and risk factors are increasing. Regardless of the economical resources that a country possesses, a well designed and well implemented national program for cancer control is able to reduce the cancer incidence and to improve the life of those that live with cancer. A complex national program for cancer control assesses different ways for the control of the disease and chooses and implement those with the lowest ratio between cost and benefits for the greatest part of population. CanScreen is an interdisciplinary project that apply IT solutions and complex mathematical theories to the benefit of society.
The major goal of the project on long term is to find an appropriate screening strategy for Romania and thus, the reduction of incidence and mortality by cancer with the improvement of quality of life for the patients. Without screening, morbidity by cancer and the treatment and diagnosis costs rise. This motivates the efforts of the scientific community to achieve the prevention of the disease and early detection by non-invasive methods.
CanScreen has the following scientifically and technological objectives:
O1: Building up of a complex fail safe system which will ensure the diagnosis and treatment quality at European Standards;
O2: Design of a database which will include the tested women and also registers with high risk women;;
O3: Design of algorithms for individualized diagnosis and a protocol for subsequent therapeutics indications;
O4: Improving the smears interpretation;
O5: Analysis and selection of smears with cytological abnormalities;
O6: Insuring the quality of the system by random selection of a percent of samples of interpreted smears and transmitting them to an expert cytologist;
O7: Distance transmission of mammography and ensuring their quality control;
O8: Designing the cost-effectiveness study which allows to choose the screening strategy with the lowest ratio of cost-effectiveness using the Markov model.
